Care Mapping Workshop – East Hawaii

Please join the Department of Education, Hilopaa and LEND for a Care Mapping workshop!

What is Care Mapping?  It is a process which guides and supports the ability of families and care professionals to work together to achieve the best possible health and educational outcomes.  It shows the needs and supports required for a child with a disability in a visual “map” that you create in real-time.  It is very helpful in determining your child’s needs and required services. It can help to create appropriate care plans, improve your child’s life, and even assist in advocacy.

In a nutshell: it will show you where your child is receiving supports and where the gaps are.
